How the dual choice option works

Affordability

One blended premium gives enrollees access to two great dental plans.


• DeltaCare USA — our dental HMO plan.
  
• Delta Dental PPO — our preferred provider organization plan OR Delta Dental Premier — our traditional managed fee-for-service plan.

Choice

• Both group purchasers and enrollees have a choice in terms of dental plans.
  
  
• First, the employer chooses two dental plans to offer — DeltaCare USA in conjunction with either PPO or Premier.
  
  
• Then, enrollees select the Delta Dental plan that best fits their needs.

Convenience

• Throughout the year, enrollees have the option to change dental plans.
  
  
• To change dental plans, enrollees simply call our special toll-free number (866) 225-1112 by the 15th of the month to change plans effective the first of the next month. Enrollees may call this same number to request additional copies of ID cards and Evidence of Coverage (EOC) booklets.
  
  
• Trained customer service representatives will handle inquiries promptly and efficiently.
